When men first hear "online pelvic floor physical therapy," a lot of them are skeptical. Pelvic floor work feels physical, hands-on, intimate. How can something like that work over video?
It's a fair question. And the answer โ backed by a growing body of research โ might surprise you.
What the Research Actually Says
Multiple studies published in urology and physical therapy journals have found that telehealth-delivered pelvic floor rehabilitation produces outcomes comparable to in-person care for post-prostatectomy patients. A 2021 systematic review found no statistically significant difference in continence outcomes between telehealth and in-person pelvic floor PT.
Why? Because the most important elements of pelvic floor rehab are instruction, feedback, and progressive programming โ all of which can be delivered effectively via video.

Access a Real Specialist
Most physical therapists are generalists. Pelvic floor PT โ especially for men post-prostatectomy โ is a specialty. In many areas of the country, there are no local pelvic floor PTs with experience in post-prostatectomy rehab. Telehealth removes geography as a barrier entirely.
Your Home Is the Right Environment
Pelvic floor exercises need to transfer to real life โ your kitchen, your stairs, your yard. Working in your own environment actually has advantages over a clinic setting. You're doing exercises in the context where they need to work.
Consistency and Convenience
Recovery takes months. The number one predictor of outcome is adherence. An online class you can join from home at 7pm is far easier to attend consistently than an in-person clinic appointment that requires driving, parking, changing, and waiting.
Small Group, Not One-on-One Only

What Online PT Cannot Do
Honesty matters here. There are real limitations to telehealth pelvic floor PT:
- Internal assessment: A manual internal pelvic floor exam โ which some PTs use to assess muscle tone, trigger points, and coordination โ cannot be done via telehealth. If you have significant pelvic pain or unusual dysfunction, an in-person assessment may be warranted first.
- Biofeedback equipment: Some in-person clinics use surface electromyography (EMG) biofeedback to help patients "see" their pelvic floor contractions. This can be helpful for men who struggle to find the right muscles. We substitute this with detailed verbal cueing and movement-based feedback.
For the vast majority of post-prostatectomy patients, these limitations don't affect outcomes. The core work โ exercise instruction, progression, and education โ is fully deliverable via video.

What to Look For in an Online Pelvic Floor PT Program
- DPT credentials: Your provider should be a licensed Doctor of Physical Therapy, not a fitness coach or wellness instructor.
- Prostatectomy-specific experience: General pelvic floor experience is not the same as post-prostatectomy specialization. The anatomy, the challenges, and the progression are different for men.
- Small groups or 1-on-1: Avoid programs that are just pre-recorded videos. Live instruction and the ability to ask questions and get feedback matters enormously for technique and safety.
- Progressive structure: Your needs at week 2 post-catheter removal are completely different from your needs at month 6. A good program is structured around your recovery phase.
